"I have been involved in Assisted Living and Skilled Nursing Facility Administration for over 30 years. I LOVE your website- the info is easy to use, and comprehensive. I typically write all of my own policies, procedures, forms etc. but since finding and joining Providers Web...I have so much more time to Direct our Operations....We have 7 licensed Homes for the Aged, Assisted Living Homes in the state of Michigan - a total of +/- 700 older persons, many with diagnoses of dementia. Just wanted to say thank you for making this information available, at such a reasonable cost!"

Free Nursing and Assisted Living Facility Forms Policies for Residential Care, Nursing Homes, ALF, RCFE, CCL

The Physician's Report

Before the actual admission of a resident to the care facility, the Physician's Report or the Health Physical as required by the state must be completed by the resident's physician prior to admission. The form should also be updated as the resident's needs change. The standard is to at least update the Physician's Report annually in assisted living facilities, RCFE's, and other community based care homes.

Excerpt:
Before the actual admission, the Physician's Report should be reviewed and checked for mistakes. Make sure the physician has completed the form properly and has signed a statement on the document that says something like: "I declare the above information is true and correct." It is vital that the facility have accurate information verified by the attending physician regarding the resident's condition and that the prospective resident is appropriate for the facility's level of care, especially if any litigation occurs...
