State Special Forms.
ProvidersWeb has provided you with a multitude of forms, which, from experience and analysis, have proven to be beneficial, cost saving, and time saving in the assisted living world. Certain states may have developed their own forms for specific situations which must be used in that state. You should be aware of your own state requirements.

To protect yourself from lawsuits, ProvidersWeb has developed many forms for internal use. Self examination, through the use of forms, is a good way to prevent events from occurring that might lead to lawsuits. Refunds.

No refunds are given if twenty (20) or more documents have been downloaded by the subscriber.

Privacy and Personally Identifiable Information.
This is governed by the Privacy Policy available on the Website under privacy policy and incorporated in this agreement as modified from time to time. Personally Identifiable Information will be protected, shared and disclosed as set out in the privacy policy.

ProvidersWeb will not disclose Personally Identifiable Information to any others unless they have agreed to be bound by ProvidersWeb’s privacy policy.

ProvidersWeb may monitor these services, but is not required to, and has the right to remove any information or content in its sole discretion. Pursuant to Title 17, United State Code, Section 512(c)(2) notifications of claimed copyright infringement must be sent to the Service Providers Designated Agent.

Applicable Law.
All actions related to these Terms and Use shall be governed by the laws of the State of California, without regard to the conflict of law provisions of any other jurisdiction. By entering into this agreement you agree to venue of any action being in the Courts located in the State of California.
