Be sure you document every person, place and thing that relates to the event, on the "Accident/Incident/Injury Report Form." Be sure you contact the resident's responsible party. This means actually talking to a contact person. Leaving a phone message isn't good enough. If you are unable to contact anyone during your shift, make sure you tell the next Shift Supervisor to continue calling until the appropriate resident's responsible party is contacted.
